This form is for Members and Visitors to request ID cards for family members for use on campus buildings and apartments. Please submit this form after your arrival at IAS.

Please select the WHO-approved COVID-19 vaccine that you received.

Date of Last Vaccine dose

Please enter the date card recipient received their most recent COVID-19 vaccination dose.

Proof of COVID-19 Vaccination (including booster shots)

Please upload proof of card recipient's COVID-19 vaccination status.

Acceptable proof of vaccination includes:

CDC COVID-19 Vaccine Card
WHO Vaccine Booklet
An official record of COVID-19 vaccination from A State immunization registry, such as the New Jersey Immunization Information System (NJIIS)
A record of COVID-19 vaccination on office letterhead signed by a licensed physician, nurse practitioner, physician’s assistant, registered nurse or pharmacist.
A military immunization or health record from the United States Armed Forces documenting COVID-19 vaccination.
